Drop-down/Reply or Select, Input, Text Area

I tried to change the drop-down menu and text entry screen for replies in my LJ. Last night it worked like a charm...this morning, I putzed with some customization in the print_my_entries override and now...Nada.

I have no idea what I did, I am sorry but I'd love someone to take a look and respond with any advice.
